If you/your child has been evaluated by another provider for your skin concern, a referral should be placed by your primary care provider or by another specialist who is involved with the evaluation and management of your skin concern. This allows our staff to ensure that we have all necessary information on your prior evaluation before we see you in our office.
The providers and staff of Kenwood Dermatology strive to provide efficient and thorough care. In order to ensure that we maximize the use of your time during your visit, we request that you come prepared with information on your medications/treatments, specific concerns, and any changes to your general health.
Depending on the complexity of your concerns, your provider will usually only be able to address from 1-3 specific concerns during your visit. If you feel as though you will need extra time, please discuss this with our office staff when you schedule your appointment.
We have a limited number of same day urgent appointments. These appointments are intended to address a single, specific urgent skin concern; require that the referring provider speak with one of our staff to clarify the concern; are triaged by one of our medical staff, and are not guaranteed.
Please complete the new patient intake form prior to your visit. The form is accessible here.
In order to prepare for your initial evaluation, we kindly request that you ensure that we have received copies of any relevant medical records prior to your visit. This includes consultation reports from any dermatologists or other specialists that you have seen, as well as copies of any biopsy/pathology reports and diagnostic testing. If we do not have this information available prior to your visit, we may not be able to provide you with a diagnosis and treatment plan at your initial visit. We will request that you assist us in obtaining this information and we will schedule a follow-up appointment for further evaluation.
If you need assistance in obtaining your medical records, please contact our office staff.
The providers and staff of Kenwood Dermatology strive to minimize your wait time in our office. When patients arrive late it can be disruptive for the staff and for other patients.
Please plan to arrive 15 minutes prior to your appointment time in order to allow adequate time for registration. If you arrive late for your appointment, we may ask you to reschedule. We recognize that unavoidable delays may occur; if you think that you may be late for your appointment, please contact our office to discuss whether we will be able to accommodate you later in the day or whether we will need to reschedule your appointment.
The providers and staff of Kenwood Dermatology work hard to provide optimal care for our patients, and we are available to you when needed. In return, we request that patients have the courtesy to cancel appointments in advance.
We require 24-hour notice of an appointment cancellation. If you do not show for a scheduled appointment and you did not provide our office with at least 24 hours notice, you will be charged a fee of $ 50. Payment of any outstanding no-show fees will be required in order to schedule another office visit. Patients who no-show and/or fail to cancel two appointments with at least 24 hours notice may be dismissed from the practice.
All deductibles, co-payments and past due balances are due at time of registration. Additional information on our financial policies may be found here.
