We schedule new patient appointments by referral only, even if your medical insurance plan does not require you to have a referral to see a specialist. This process ensures that we are the appropriate specialist to address your medical concerns. As part of the referral process, our staff will clarify the reason for referral, request copies of relevant medical records and a summary of your health history if not provided by the referring provider, and provide communication to your primary care provider and/or other medical specialists regarding your plan of care.
If you are a new patient or parent who wishes to schedule an appointment, please contact your primary care provider to request a referral. Please note that we are unable to accept a referral from an emergency room or urgent care.

New patients, please also complete the new patient intake form and bring the completed form to your appointment. Having the intake form completed prior to your visit will permit the medical staff to provide the most efficient care during your visit.
The providers and staff of Kenwood Dermatology strive to provide efficient and thorough care. In order to ensure that we maximize the use of your time during your visit, we request that you come prepared with information on your current medications, prior medical treatments, specific concerns, and any changes to your general health. If you have had any skin biopsies or if you have seen a dermatologist or other specialist for your skin condition, it is preferable that you request these medical records prior to your appointment.
Depending on the complexity of your concerns, Dr. Shah will usually only be able to address from 1-3 specific concerns during your visit. If you feel as though you will need extra time, please discuss this with our office staff when you schedule your appointment.
Please do not wear foundation or other make-up to your visit; this is especially important if you have been referred for evaluation for acne or for a mole check or skin cancer screening.
If you are a provider wishing to refer a patient, you may do so by faxing a referral request to 513-745-5515. In order to expedite the referral process and to ensure that patients receive optimal care, we request that pertinent medical records (e.g. office notes, pathology and other laboratory results, and specialty consultation reports) as well as a health summary (e.g. medications, allergies, medical and surgical history) accompany the referral request. Medical records may be faxed to the office at 513-745-5515 or emailed to the office at email@example.com. Please ensure that medical records are sent securely.
We currently provide care predominantly for pediatric, adolescent and young adult patients.
Dr. Shah also sees adult patients for concerns such as acne, psoriasis, and skin cancer screening.
We do not currently provide cosmetic procedures such as laser hair removal, Botox or fillers.
We accept many major medical insurance plans.
We do accept Medicare at this time.
