Make a Payment
If you wish to make a payment on your account, please provide the following information. You will then be re-directed to our merchant services provider, Pay Junction, to complete your payment transaction.
Please have your credit card information and invoice ready.
Please use the correct patient name in the field below.
Please include the patient date of birth and the account number from the billing invoice in the notes section of the payment transaction page, which will load after you select “Make a Payment.”
